Basic DS-11 Application (Bring own photo and all required documents)

Cost displayed is per applicant. In the case of more than three total applicants, please make two bookings.
NO DS-82 Adult Renewal OR OTHER application forms can be accepted as stated by the U.S. Department of State.
MUST bring: Valid photo ID; certified citizenship document; acceptable 2 x 2 passport photo.
Optional prior to arrival: Photocopies of ID and citizenship documents; completed DS-11 Application form.
We will provide: Photocopy services as needed; DS-11 Application form as needed; Execution of DS-11 application.

Full DS-11 Application (Photo taken at UT Tyler, you bring required documents)

Cost displayed is per applicant. In the case of more than three total applicants, please make two bookings. Please ONLY notate the number of APPLICANTS in the appropriate line under "# of minors" and/or "# of adults". We use the numbers here to provide fees due.

Must bring: Valid Photo ID; Certified Citizenship Document. Proper forms of Pay - DOS and UT Tyler fees CANNOT BE COMBINED.

OIP is authorized by the U.S. Department of State to execute and mail in Form DS-11 passport applications. We cannot review or mail in DS-82 renewal applications, and are limited to only providing the blank application form and paid photo services.
